1. A copy of your new Polaris 3900 SPORT sales receipt pre-printed with the participating dealer name..
2. The completed 3900 SPORT warranty extension card. (If the card has been sent separately, please include a copy of the card.)

4. The main body (no hoses, wheels or pumps) of your old automatic pool cleaner.

It is not necessary to take any extraordinary precautions when packaging your trade-in cleaner. A broken cleaner will not affect your trade-up eligibility. We suggest putting items 1, 2, & 3 in an envelope and taping it to the old cleaner as an added precaution. We recommend that you send the cleaner FedEx and save all postal receipts in case a trace becomes necessary. Please note: Polaris needs to receive these items no later than September 30, 2011. Polaris Pool Systems, inc. will mail your check within one week of receiving all four items.
